VINCENT ISAAC
PHYSICAL THERAPIST
Vincent Isaac is an energetic and passionate physical therapist specializing in orthopedics and sports medicine. As the Clinical Director of FreeForm Physical Therapy’s Lincoln Park office, he also leads the company’s Clinical Education program, ensuring the highest standards of care and professional development.
Dr. Isaac earned his Bachelor’s degree in Biology from the University of Illinois-Chicago before obtaining his Doctorate in Physical Therapy from A.T. Still University in 2014. Committed to continuous growth, he completed a post-graduate orthopedic residency, further refining his hands-on skills and clinical expertise.

Dr. Isaac specializes in treating back pain, upper and lower extremity conditions, post-surgical rehabilitation, and athlete recovery. He is also TPI-certified which gives him in-depth knowledge on golf specific rehabilitation. He is dedicated to evidence-based, patient-centered care, tailoring treatment plans to each individual’s needs. With a holistic approach, he helps both athletes and non-athletes move better, feel better, and prevent future injuries—all while empowering them to achieve their health and performance goals.
DENIS O’CALLAGHAN
PHYSICAL THERAPIST
Denis O’Callaghan received his Bachelor’s degree in Kinesiology from the University of Illinois at Urbana-Champaign and his Physical Therapy Degree from Governors State University. Prior to earning his Physical Therapy degree, he worked as an assistant Track and Field coach working with high school athletes in the Chicago suburbs.
While at the University of Illinois, Dr. O’Callaghan competed for the men’s Cross Country and Track and Field teams, where he was a multiple time team scorer in the Big Ten Conference and earned one school record. He utilizes his expertise in athletics (along with his own experiences dealing with injury throughout his athletic career) to guide his patients down the road to recovery by optimizing human movement and strength.
Dr. O’Callaghan has spent his entire career thus far working in sports medicine and orthopedics. His goal is to establish a whole-body approach to treatment in order to develop a plan specific to each individual and their goals. He also has experience working with vestibular conditions and utilizing the McKenzie method. Dr. O’Callaghan uses a comprehensive, hands-on approach with each patient to maximize function, movement, and performance across all domains of life.
ARSHYA ADELI
PHYSICAL THERAPIST
Arshya Adeli is a dedicated Physical Therapist with a passion for optimizing movement and enhancing performance. After earning her Doctorate of Physical Therapy from USC in 2003, she has become a prominent figure in the field of orthopedics and sports medicine.
Armed with a diverse skill set, Dr. Adeli is certified in various specialized techniques, including trigger point dry needling, Delfi blood flow restriction therapy, Graston, cupping, instrument-assisted soft tissue mobilization, and the McKenzie method. Her commitment to staying at the forefront of rehabilitation practices ensures that she delivers cutting-edge and effective treatments to her patients.
With a profound background in dance, particularly ballet, Dr. Adeli brings a unique perspective to her treatment approach. She has a comprehensive understanding of movement intricacies and the demands placed on the body, especially in high-performance settings. She has extensive experience working with athletes performing at recreational to elite levels.
Beyond her clinical work, Dr. Adeli is deeply committed to ongoing education, professional mentoring and community engagement. She remains dedicated to empowering individuals to achieve their optimal physical well-being. With a career spanning over two decades, Dr. Adeli continues to exemplify excellence in physical therapy and continues to make a meaningful impact on the lives of those she serves.
OMER AGUILAR
PHYSICAL THERAPIST
Omer Aguilar has spent his entire career in orthopedics and sports medicine since completing his Physical Therapy Degree at Wayne State University in Detroit, MI in 2001. He went on to complete his postgraduate training with the North American Institute of Manual Physical Therapy to further his hands-on and clinical skills. He has attained certifications in Trigger Point Dry Needling, Active Release Techniques and instrument assisted soft tissue mobilization. Completing multiple marathons and triathlons has uniquely developed his understanding of effectively treating the endurance athlete. Dr. Aguilar is an expert working with athletes and non-athletes of all levels with the understanding that effective treatment requires a whole-body approach.
MEGAN NG
PHYSICAL THERAPIST
Megan Ng earned her Bachelor of Science degree in Exercise Science followed by a Doctorate in Physical Therapy from Saint Louis University. She also competed for the university's Cross Country and Track and Field programs where she also served as team captain.
Dr. Ng’s experience covers an array of outpatient orthopedic settings treating individuals for orthopedic injuries, post-surgical rehabilitation and deconditioning. In practice, she focuses her approach with an empathetic and problem-solving mindset to best curate a plan of care focused on an individual’s personal goals. She is committed to addressing a patient’s movement system for greatest functional gains. As a former NCAA Division I athlete, Dr. Ng has the unique ability to use her clinical skills to treat the athlete at any level. She is enthusiastic about maintaining a holistic approach that encompasses prevention, rehabilitation and maintenance care.
